If you are paying by credit card, you have two options
for verification:
1. You may visit our offices in Richardson, TX and
we will make an imprint of your card
2. You may print out the Credit Card Authorization
Form by clicking on the button below
If you choose option 2, please remember that all fields
in the form need to be filled out. After you have
filled out the form, attach a photocopy of the front
and back of your credit card and a photocopy of your
State Drivers License or National Passport and fax
all three pages to us at 972-479-0523.
